High Country Early Intervention

How to Obtain Services for Your Child in Arizona

A) Arizona Early Intervention Program (AzEIP) Criteria: A child from birth to 36 months of age will be considered to exhibit developmental delay when that child has not reached 50 percent of the developmental milestones expected at his/her chronological age, in one or more of the following domains:
 
Physical: fine and/or gross motor/sensory (includes vision and hearing)
Cognitive  
Language/communication  
Social or emotional  
Self help/adaptive  
     

Established conditions, which have a high probability of developmental delay include, but are not limited to:

Chromosomal abnormalities Periventricular Leukomalacia
Metabolic disorders Cerebral palsy
Hydrocephalus Significant auditory impairment
Neural tube defects (spina bifida) Significant visual impairment
Intraventricular hemorrhage Failure to thrive
  Severe attachment disorder

B) AzEIP: Department of Developmental Disabilities (DDD) Criteria:

The child must already be receiving services from DDD or have been referred to a DDD support coordinator.
The child must be between 0 - 3 years of age.
The child must be determined to be DDD AzEIP or Title 19 eligible.

C) ADHS: AzEIP

Children with special health care needs.
Children with special health care needs, eligible for AzEIP services.
Survivors of traumatic brain injury (TBI).

D) ASDB: AzEIP

The child must be between 0 - 3 years of age.
The person having legal custody of the child is a legal resident of Arizona, and
The child is sensory impaired due to a hearing or vision impairment to an extent that the child would benefit from a specialized program.

How to access services:

If you have any concerns related to your child's development, you can always contact us at High Country Early Intervention (928) 777-9525. To directly refer your child for an evaluation, contact Barbara Kramar at AzEIP 1st! (928) 759-5591. AzEIP 1st! will schedule an intake visit with you to discuss the need for a developmental evaluation/assessment for your child at a time and place that is convenient for you. Following this evaluation, it will be determined if your child is eligible for services under DES: AzEIP, DES: DDD or ADHS/AzEIP.

If your child is eligible under DES: AzEIP, a referral will be made by AzEIP 1st! to High Country Early Intervention. This will take place at the time of the initial IFSP.

If your child is eligible under DES: DDD or ADHS/AzEIP, a referral will be made to the appropriate agency that will assist you in securing service for your child and family.
