How to access services:
If you have any concerns related to your child's
development, you can always contact us at High Country Early Intervention (928) 777-9525. To directly refer your child for an evaluation,
contact Barbara Kramar at AzEIP 1st! (928) 759-5591. AzEIP 1st!
will schedule an intake visit with you to discuss the need for
a developmental evaluation/assessment for your child at a time
and place that is convenient for you. Following this evaluation,
it will be determined if your child is eligible for services under
DES: AzEIP, DES: DDD or ADHS/AzEIP.
If your child is eligible under DES: AzEIP, a referral
will be made by AzEIP 1st! to High Country Early Intervention. This will take place at the time of the initial IFSP.
If your child is eligible under DES: DDD or
ADHS/AzEIP, a referral will be made to the appropriate agency
that will assist you in securing service for your child and family.
